Job Summary

The Community Connectors / Link workers provide support and advice to Blaenau Gwent citizens and their families and carers. They promote independence and healthy lifestyles, preventing social isolation and increasing citizen’s personal wellbeing.

This post can also be offered as a secondment opportunity. Before applying for this as a secondment opportunity, candidates should seek approval to be released from their current post by their Line Manager. Secondments are available to permanent staff of the Authority only.

The Community Connectors / Link workers provide support and advice to Blaenau Gwent citizens and their families and carers. They promote independence and healthy lifestyles, preventing social isolation and increasing citizen’s personal wellbeing.

The overall purpose of the Community Connector (CC)/ Social Prescribing link role is to support the Council and wider strategic partners (including Health) to develop innovative strategies that enable citizens to enjoy more independent lives in their communities.

Link roles (Community Connectors / Social Prescribers) provide important connections between people, professionals and the wider well-being network. They spend time with individuals to find out what matters to them, co-produce solutions and link people with non-clinical community based support.

We are looking for additional 4 Community Connectors to join our existing team to enable the service to develop further and include designated links to our community of GP practices.

The successful applicant will need to be educated to at least NVQ Level 3 or A Level Standard or equivalent standard/professional qualification in social care, health or community development, the successful applicant will possess:

  • A good understanding of asset-based approaches to community development and individual well-being
  • Knowledge and understanding of relevant legislation for health and social care
  • Understanding of citizen, family and carers needs.

The position carries an Approved Casual Car User Allowance therefore access to a vehicle for work purposes is a requirement.

Thorough pre-employment screening is undertaken during the recruitment process. The post holder will be required to complete an application for a Disclosure of Criminal Records.
