Full AdvertAn exciting opportunity has arisen within the Adult Services Division of Blaenau Gwent County Borough Council for a qualified Social Worker. We are looking for an enthusiastic individual to join our team and develop their skills.The successful candidate will undertake the assessment and care management process with vulnerable adults, including identification of risks, in accordance with current policies and procedures of the authority.You will be required to implementation of individual service user and / care outcomes and monitor the progress in achieving these outcomes and goals, as well as empowering service users to achieve their potential for independence and where appropriate their choices to be met.There will also be the requirement to work alongside colleagues from statutory and voluntary agencies, through multi-disciplinary assessments and meetings and to promote awareness among carers of their entitlement to assessments and where required to undertake those assessments.You must be registered with the Care Council for Wales and possess a Diploma / Degree in Social Work. Previous experience of working in Health and / or Social Care setting as well as knowledge and understanding of the Mental Health Measure including Complex Care Management, Continuing Health Care and recent legislation and good practice in Adult Community Care is essential.The position carries an Approved Casual Car User Allowance therefore access to a vehicle for work purposes is a requirement.Thorough pre-employment screening is undertaken during the recruitment process.
