Job Summary
 
The overall purpose of the Community Connector (CC) role is to support the division to develop innovative strategies that enable citizens to enjoy more independent lives in their communities.
 
This post is open to permanent, fixed term and temporary (with 12 months continuous employment) employees of BGCBC only.

An exciting opportunity has arisen for experienced individuals to provide support and advice to vulnerable adults, their families and carers that enables citizens to enjoy more independent lives in their communities, preventing social isolation and increasing wellbeing.
 
The successful candidate will be required to develop effective community links and networks and promote partnerships to create community based solutions that will:
 
a)    Increase an individual’s ability to have choice and control over the way they receive support
b)    Support empowerment through information, advice and local opportunities
c)    Improve social and emotional wellbeing through developing opportunities within the local community
d)    Reduce social and emotional isolation and loneliness, by promoting independence of service users
                                        
The successful candidate will be required to have good communication skills in order to work alongside colleagues from statutory and voluntary agencies, through multi-disciplinary assessments and meetings.
 
Educational attainments for the post include NVQ Level 3, A Level standard or an equivalent standard qualification in social care, health or community development. Previous experience of working in Health and/or Social Care settings as well as experience of providing and delivering training / briefing sessions to staff / colleagues is essential. You will also have good knowledge and understanding of relevant legislation for health and social care as well as experience of working with a wide range of organisations.
 
The position carries an Approved Casual Car User Allowance therefore access to a vehicle for work purposes is a requirement.
 
Thorough pre-employment screening is undertaken during the recruitment process. The postholder will be required to complete an application for a Disclosure of Criminal Records.
