Job SummaryTo assist in delivering an effective and efficient Independent Living service in Blaenau Gwent. Independent Living Officers are expected to support and motivate staff to deliver a personalised service, in partnership with service users. They are responsible for ensuring the service enables individuals to maximise their independence, achieve their agreed outcomes and meet CSSIW domiciliary care standards.This post is open to permanent, fixed term and temporary (with 12 months continuous employment) employees of BGCBC only.Full AdvertYou will be provide information, advice and support to service users and their families relating to the provision of the Home Care Service and continually monitor service provision, ensuring principals of independence are achieved.You will monitor and supervise support staff and assess, update and regularly check risk assessments, ensuring they are carried out in relation to both carer and service user. You will review the effectiveness of support with the service user and staff and promote and maintain a safe working environment.You will be required to take a multi-disciplinary approach to case management, attending case conferences/joint reviews when required and to work with Care management staff to identify, deliver and monitor appropriate support plans that reflect individual service users’ outcomes. You will identify and co-ordinate the involvement of specialist services such as District Nurse; Therapy staff; Continence Advisor; Community Psychiatric Nurse; Housing staff; social worker etc. where appropriate for the service user.Essential to the role is a NVQ 3 in Care or Management gained in a Care setting, a Manual Handling All Wales Passport and commitment to further qualifications. Significant relevant working experience in the social/health care sector is required as is experience in supervising staff, working with other agencies and understanding and respecting the principals of confidentiality. Knowledge of risk management, CSSIW Standards, the needs of service users and the Promoting Independence Agenda is also essential.The position carries an Approved Casual Car User Allowance therefore access to a vehicle for work purposes is a requirement.Thorough pre-employment screening is undertaken during the recruitment process.
