Job Summary

 

The post holder will be responsible for the assessment and care management service for vulnerable adults in the community.

An exciting career opportunity has arisen within Adults’ Services for a qualified Social Worker.

 

The successful candidate will undertake the assessment and care management process with vulnerable adults, including identification of risks, in accordance with current policies and procedures of the authority.

 

You will support service users in living independently in their communities and promote services which help avoid unnecessary hospital admissions and facilitate early discharges from hospital. This may require working as part of a discharge team at weekends.

 

There will also be the requirement to work alongside colleagues from statutory and voluntary agencies, through multi-disciplinary assessments and meetings and to promote awareness among carers of their entitlement to assessments and where required to undertake those assessments.

 

You must be registered with the Care Council for Wales and possess a Diploma / Degree in Social Work. Previous experience of working in Health and / or Social Care setting as well as knowledge and understanding of Complex Care Management including Continuing Health Care and recent legislation and good practice in Adult Community Care is essential.

 

The position will be reviewed toward the end of the fixed term as there may be an opportunity to extend beyond the initial 6 months.

 

The position carries an Approved Casual Car User Allowance therefore access to a vehicle for work purposes is a requirement.

 

Thorough pre-employment screening is undertaken during the recruitment process. The postholder will be required to complete an application for a Disclosure of Criminal Records.
