A secondment opportunity has arisen within the Contracts and Commissioning Team for a professional, enthusiastic and confident individual.  You will need to work effectively both as part of a busy team and alone, using your own initiative, developing and building upon good working relationships with all internal and external stakeholders. 

You will manage brokerage processes via established systems for capacity mapping, suspended packages, hospital admissions and discharges and the ‘call-off’ procedure, whilst continually striving for new ways of improvement.  You will need to be able to initiate and implement creative solutions to ensure vulnerable individuals are supported with an appropriately commissioned care package when capacity within the market is limited.  Your skills will enable you to complete necessary associated documentation; creation, maintenance and monitoring of service provisions on WCCIS; production of Individual Service Contracts and manage statistical data on Excel spreadsheets. 

You will support a designated Adult Community Care Team and area of the borough, however, there is a need for flexibility as you will be required to cover both Adult Community Care Teams and the whole of the borough when the opposite post-holder is absent.

You will also work closely with Contract Monitoring Officers to assist with performance reviews and contract compliance of providers of commissioned services in both the private and voluntary sectors to ensure that the best quality of service possible is commissioned for the citizens of Blaenau Gwent.

You will need to have a sound knowledge of the Social Services Wellbeing (Wales) Act 2014, Regulation and Inspection (Wales) Act 2016 and the Wales Safeguarding Procedures (2019) in order to identify concerns and ensure that these are acted upon and reported to the relevant Officers.
