Blaenau Gwent is looking for a dynamic, innovative and inspirational team manager to lead their Community Resource Team (CRT) – part of the Gwent Frailty programme.
As an experienced qualified social worker, nurse, therapist or allied health professional, you will manage the CRT health and social care team which is part of the newly created Blaenau Gwent Long Term - Care and Support portfolio.
You will be based within the Adult Services division of Social Services but also work in close partnership with the Blaenau Gwent ABUHB locality team including our CRT rapid and medical teams and as a part of the Blaenau Gwent Neighbourhood Care Network (NCN).
You will also work closely with our ABUHB community and acute hospital sites, our housing partners and our third sector agencies. As team manager you will have responsibility for line managing:
- Therapy team including specialist falls and reablement support
- Hospital discharge hub / home first service
- Independent living support including sensory assessments, specialist aids and equipment, assistive technologies and CARIAD, the Blaenau Gwent reablement (step up/ down) service.
- Other newly developed integrated CRT services – subject to the outcome of the ongoing care of the elderly (COTE) and Gwent Frailty review – due to be completed by December 22.
