Key duties of the team include assessment and care and support planning for children and young people in need of care and support and also safeguarding children and adults who are at risk/suffering abuse/harm.
The ethos of the team is to promote and develop children and young people’s independent living skills and to support them to live as independently as possible. This is achieved from practicing from a strengths and outcome focused approach which is well embedded across the team.
In addition to supporting children and young people, the team also completes carers assessments, seeking to explore and identify with parent/carers their own personal outcomes and how, what support they may require, to work towards achieving these.
Supporting young people up to the age of 25 is unique in terms of social work practice and allows workers in the team to support young people through their transition journey into adulthood, working in partnership with the young person, their parent/carers and partner agencies to promote a smooth transition.
Workers in the team hold a mixed case load across the age range of 0-25 years of age; they have a wide range of skills, knowledge and experiences and they are keen and willing to welcome and support new workers who share their passion and commitment to supporting children, young people and their families/carers to achieve their identified outcomes. Blaenau Gwent Disability (0-25) Team is a strong, supportive work environment with a collaborative team ethos.
All workers in the team have regular, formal supervision, at least once a month. The team also has regular, at least monthly, group supervision, which provides the opportunity for workers to reflect on cases within a safe supportive space. The team also uses this time to explore different direct work tools and theories to inform interventions and share good practice.
In terms of career development, the post is an excellent opportunity to learn and develop and gain experience in terms of working with children and young people/adults.
