We have an incredible opportunity available in our Blaenau Gwent Disability (0-25) Team for a secondment/fixed term, Full Time, Support Worker. This is a rare and exciting chance to join a team that is unique in its support for children and adults with a disability aged 0-25 and their families/carers.
As a member of our team, you will have the opportunity to play a key role in supporting young people transitioning to adulthood and working in partnership with other agencies to promote a smooth transition. You will be responsible for assessing and care and support planning for children and young people in need of care and support, as well as safeguarding children and adults who are at risk/suffering abuse/harm.
At the heart of our team is a commitment to support children and young people with a Disability (0-25) and their families/carers to identify and work towards achieving their personal outcomes and to develop their skills and confidence to live as independently as possible. You will be part of a team that practices from a least restrictive, strengths and outcome-focused approach when working with children, young adults and their families.
Our team members hold a mixed case load across the age range of 0-25 years of age and are passionate and committed to improving outcomes for all those they support. While experience across the age range is desirable, it is not essential. What is important is that you have the relevant skills, knowledge, and experience of working with either/children or young adults as outlined in the job description.
We are looking for candidates who are passionate about the development of children and young adults with a disability and who are eager to learn and grow in this area of social work practice. Blaenau Gwent Disability (0-25) Team is a supportive work environment with a collaborative team ethos.
As a member of our team, you will receive regular formal supervision at least once a month. We also provide regular group supervision, which provides the opportunity for workers to reflect on cases within a safe and supportive space. During these sessions, we explore different direct work tools and theories to inform interventions and share good practice.
